Nursing Home Violation

A Champaign nursing home is fined by the Illinois Department of Public Health.

The department cited Illini Heritage Rehab and Health Care with an A-violation, meaning there's a considerable probability that death or serious mental or physical harm will occur at the facility.

Illini Heritage Rehab was fined $25,000 for failure to implement and maintain fall interventions.

Officials say it resulted in one resident with a fractured spine.

They say the facility did not request a hearing and will pay a reduced fine of $16,000.
